Análise comparativa entre localização com semente radioativa e localização com fio metálico para cirurgia conservadora de câncer de mama não palpável

Resumo

A cirurgia conservadora do câncer de mama não palpável requer um método de localização intraoperatória que oriente a identificação e a correta excisão da lesão. O objetivo deste estudo é analisar comparativamente duas tecnologias de localização intraoperatória mamária, localização guiada por fio (WGL) e localização de sementes radioativas (RSL), quanto à sua eficácia cirúrgica acerca dos desfechos de margens cirúrgicas, reexcisão intraoperatória, reoperação e recorrência. Para tanto, foi realizada uma busca sistemática em bases de dados por ensaios clínicos que atendessem aos critérios de elegibilidade desse estudo. Os artigos selecionados foram avaliados quanto a sua qualidade metodológica e os dados foram então coletados e sintetizados quantitativamente. Os resultados compreenderam trinta e oito estudos que atendem aos critérios de elegibilidade. Os principais desfechos relatados demonstram que o RSL é pelo menos equivalente ao WGL em taxas de eficiência cirúrgica, o que confirma a aplicabilidade do método para cirurgia de lesões impalpáveis ​​de mama de forma eficaz, além de apresentar otimização organizacional dos serviços de radiologia e cirurgia ao permitir que a cirurgia seja realizada até dois meses após a implantação da semente.
